Dr. Tresch, Yvette Klonisberg and Dr. Whitney Fujii-Doe joined the coach call
Tresch has been reviewing materials in DyCEP platform and finding it really useful- noting appreciation for how it is setup and organized- asked if it was ok to use these materials- I noted absolutely. Working on injectable buprenorphine and had questions about Brixadi. Was not able to enter a new discussion topic- Lisa added a Brixadi topic on their behalf and messaged team to fix the issue in DyCEP. Discussed need for patient facing materials about different buprenorphine formulations and that it is ok to move between them- could include methadone transfer information. Discussed work camp to jail patients and need to ensure medications are setup. Still would like to continue the change project on connection to the community- working on identifying top referral sources and possible ways to integrate pharmacy injection clinics (as a bridge clinic of sorts for medication continuation). Challenge is still in having permanent community providers to continue treatment long term. review change project form- suggested adding PDSA cycle info for connection to the community providers and tracking referrals made and received from community providers. Discussed challenges with methadone access- CHAMP clinic on Maui - plan to connect with them to support methadone referrals for patients who wish to change medication or return to methadone-n next meeting 6/25 11:00
Yvette Konisberg hopes to set up systems similar to Oahu in Maui
Tori noted she participated JCOIN in cohort C- Hawaii correctional system has multiple institutions across the islands - looking to expand to other facilities
Liane suggested the team focus on the handoff to the community since there is an existing flow to get someone connected (can also work on process in the jail as well and even do another change project)
Discussed community services options - no Sublocade providers - but there is a program that allows pharmacies on Maui to have pharmacists administer Subocade - waiting on more info from pharmacy rep
Navigator services Maui Aid with incentive program (cash/gift card) for insurance and MOUD; harm reduction organization in Oahu working on a Suboxone provider on Maui
